What is mesotherapy for hair?
Mesotherapy for hair is an outpatient technique born in the 1950s in France and adapted to trichology. It consists of microinjections directly into the scalp, at the level of the superficial dermis. The needle - usually 4 mm - injects a cocktail of substances chosen based on the type of hair loss and the patient's response.
The mix may include:
- B vitamins, biotin, zinc for follicle metabolism
- hyaluronic acid for hydration and microcirculation
- aminophylline or carnitine to improve local microcirculation
- drugs such as minoxidil, finasteride or dutasteride (at low doses and only by prescription)
The needle penetrates 1-2 mm - just into the epidermis. No anesthesia is needed: many patients tolerate the discomfort. A session lasts 15-30 minutes, depending on the area to be treated. The typical protocol involves 4-8 sessions, one every 7-14 days. After that, monthly maintenance is carried out for at least 6 months.
Unlike PRP (which uses only the patient's plasma), hair mesotherapy delivers external active ingredients. This makes it more flexible: the mix can be modulated according to the phase of the hair follicle cycle. It works best on diffuse hair loss or early thinning - not on areas that have been bald for years. Living follicles respond to the stimulus; dead ones do not.
In practice, after 3-4 sessions a reduction in hair loss is seen. Regrowth occurs after 4-6 months, with thicker and less fragile hair. Results last as long as maintenance continues: stopping means losing the benefit within 3-6 months.
The author has seen it work on men and women between 25 and 50 years old with androgenetic alopecia at stage II-III of the Hamilton scale. It is a minimally invasive procedure with no downtime - you can return to work immediately. It costs between 80 and 150 € per session in Italy, but abroad it can be found at half the price.
Does hair mesotherapy really work?
The question everyone asks. Does it work or not?
The numbers give a clear answer. Out of 100 people who complete a cycle of 8-10 sessions, about 55-65 see a reduction in hair loss within the third month. One in two patients also notices a visual thickening of the hair, not just under a microscope. This is because the injections deliver vitamins, amino acids, and sometimes minoxidil directly into the dermis, where the bulb needs them most. Blood only reaches so far. Hair mesotherapy bypasses intestinal absorption and liver filtration. The active ingredients go straight to the scalp.
But be careful. It is not a magic wand. On stable baldness, with areas already shiny for years, nothing grows back. The bulb must still be alive, only in a slowing phase. Reputable centers say this immediately, without promising miracles. If the follicle is dead, the syringe is useless.
A 2022 study on 40 men compared mesotherapy with placebo. After six months, the treated group had lost 40% less hair than the control. Nothing stratospheric, but the difference was statistical and visible. Similar data come from Turkish and Italian clinics. The literature is not extensive, but it exists.
The mechanism is simple: the cocktail of active ingredients unlocks the growth cycle. The anagen phase lengthens, the telogen phase shortens. Hair falls out less, stays attached longer. That is why an increase in density is seen after a few months, not before.
It must be said clearly: it does not replace a transplant. If you have diffuse thinning but the bulb is still active, mesotherapy can be an excellent first step. If you have completely smooth areas, something else is needed. Many clinics offer it together with oral therapy or PRP, not as an alternative.
My opinion? It works, but with the right doses and consistency. Improvised centers use standard vials without personalizing. Good doctors choose ingredients based on your scalp, type of hair loss, and response to initial treatments. The difference between an effective regenerative treatment and a waste of time lies there.
How is the mesotherapy treatment carried out?
The session begins with a preliminary visit. The doctor examines the scalp, evaluates the thinning, and discusses expectations. If the patient is an ideal candidate, we proceed. No downtime.
Before the injections, a local anesthetic cream is applied. You wait about twenty minutes for it to take effect. Without anesthesia, it would not be tolerable: we are talking about hundreds of micro-punctures. With the cream, most patients report only mild discomfort.
The doctor prepares the mesotherapy cocktail. The composition varies depending on the specific case: hyaluronic acid to hydrate the bulb, B vitamins for hair metabolism, minerals like zinc and copper, and often essential amino acids. Sometimes PRP (platelet-rich plasma) is added, taken from the patient's own blood. The choice depends on the diagnosis: there is no universal formula.
The injections are made with very thin needles. The doctor proceeds in small areas, spacing each point about one centimeter apart. They focus on the thinnest areas: vertex, temples, parting line. The depth is minimal, a few millimeters, just into the superficial dermis. Each session lasts between thirty and forty minutes. There is no need to stop working: you leave and return to your activities. At most, some local redness that disappears in a few hours.
For PRP, blood is drawn just before the treatment. A special tube and a centrifuge are used. In about ten minutes, the plasma is ready. It is injected using the same technique as hair mesotherapy, but often the two methods are alternated in different sessions.
After the session, no hot showers or washing for 24 hours. No sauna, direct sun, or activities that cause sweating. The scalp should be left alone. Small scabs may appear at the injection points: they disappear in a couple of days.
The regenerative treatment involves a cycle of 4-6 sessions, spaced 10-15 days apart. Results begin to appear after about three months, when the cycle is completed. But the first session itself is a quick and safe outpatient procedure.
How much does mesotherapy for hair cost and how long does the effect last?
Let's start with a concrete fact: a basic cycle of hair mesotherapy costs between 200 and 400 euros in Italy. The final bill increases if the doctor uses richer cocktails of active ingredients, such as compounds with growth factors or specific trace elements. A single session at a mid-level center in Milan or Rome runs around 60-100 euros. In the South, you find lower prices, 40-70 euros per session.
Why the difference? It depends on three variables. The injected substance: basic mixtures with vitamins and amino acids cost less than products with platelet-rich plasma or targeted anti-inflammatory ingredients. The number of sessions: standard protocols include 4-6 initial sessions on a weekly or bi-weekly basis. After the initial cycle, a maintenance session is needed every 3-4 months. The third factor is the doctor's expertise. A dermatologist specialized in trichology charges higher fees than a general beautician - but on the scalp, it's better not to skimp.
A quick cost comparison
PackageAverage costSessions included Basic (vitamins + minerals)€250-3504 sessions With enhanced active ingredients€350-5004-5 sessions Premium (with growth factors)€500-8005-6 sessionsThe maintenance months are the expense many forget. After the initial cycle, a maintenance session every three months costs 60-100 euros each time. Over a year, factor in an extra 240-400 euros. In practice, in the first twelve months, you spend between 500 and 1,200 euros to achieve and protect the result.
How long does the effect of a session last? It depends. The visible improvement lasts on average 3-4 months if the thinning is mild. On already compromised hair treatment, the effect shortens to 6-8 weeks. Don't expect miracles: mesotherapy does not regrow dead follicles. It works if there is still follicular activity to support. In one of the centers I visited in Istanbul, a dermatologist explained that 30% of patients see a clear result by the third session, while the rest notice gradual changes around the fifth or sixth.
The duration also depends on consistency with maintenance. Those who follow the maintenance plan retain the result for 18-24 months. Those who skip maintenance return to the starting point in 4-5 months. The cost of hair mesotherapy is an investment that needs to be renewed, not a one-time solution.
A direct tip: before signing a package, ask the doctor for a written quote separating the initial cycle from maintenance.
Contraindications and side effects of mesotherapy
Hair mesotherapy is not a procedure for everyone. There are conditions that completely advise against it and others that require careful evaluation. Before starting, the doctor performs an examination to rule out absolute contraindications:
- Pregnancy and breastfeeding - due to lack of studies on the safety of the injected active ingredients.
- Known allergy to one or more components of the cocktail (hyaluronic acid, vitamins, minerals, local anesthetics).
- Infections or open wounds on the scalp (dermatitis, active psoriasis, bacterial folliculitis).
- Active systemic autoimmune diseases (lupus, scleroderma) - the risk of inflammatory reaction is too high.
- Oral anticoagulant therapy (warfarin, DOACs) - bleeding during injections is uncontrollable.
There are also relative contraindications: those with uncontrolled diabetes or a history of keloids should discuss it first with the trichologist. In any case, the final decision rests with the doctor.
The most common side effects of mesotherapy are local and temporary.
