Thinning temples: the right question to ask
Those who arrive at the question "how many grafts for the temples" have usually already looked in the mirror for months, and have noticed that the hairline has receded at the sides. It's a specific point: the temporal area doesn't thin like the crown, it doesn't lose hair uniformly but recedes with a sharp, often symmetrical profile that frames the face. The right question, however, isn't the number itself. It is: how many grafts do you need for your temple, with your surface area to cover and your starting density. A serious surgeon won't answer "500 grafts" over the phone. First, they measure. The classic temporal area ranges from 5 to 20 cm² per side, and the implant density is deliberately kept lower than the crown: you don't need to thicken it like a forest, you need to rebuild a natural edge. In practice, for mild thinning you start with 300-400 grafts per side. for a completely empty temple you reach 700-800. If the temples need to be rebuilt together with the frontal hairline, the total rises and we're talking about 1,500-2,500 grafts distributed. Concrete data helps to understand why centimeters matter, not numbers: a post-operative density of 25-35 follicular units per cm² is considered physiological. With a defect of 8 cm², about 240-280 grafts are needed just to even out the area. Below that threshold, the "transparency" effect remains visible. The temptation to ask "how many does one cost" is wrong for another reason: temples have hair that moves downward and forward, and the ductility of the graft makes the difference. The same number, extracted from a fragile donor area, yields much less. The evaluation should be done by looking at three things:
- The actual surface area to cover, measured not by eye but with trichoscopy.
How many grafts are needed for the temples? The factors that change everything
The question "how many grafts are really needed for the temples" doesn't have a single answer, but the numbers that come up most often in consultation are clear: from 400 to 700 units per side. In more advanced cases, you reach 900-1000 per temple. They seem few, compared to the 2000-3000 grafts of a full forehead. In reality, they are many, because the temporal area is small and each graft must work at a precise angle.
The surface area to cover
The first factor is the extent of the area. Rebuilding only the anterior corner, the one that frames the eye, is a job of 250-450 grafts per side. Redoing the entire temporal region, up to the line that rises toward the crown, means going up to 700-1000 per side. The difference between "fixing the corners" and "redesigning the temple" is enormous.

The second factor is the hair caliber. A thick shaft, around 80-90 microns, covers more with fewer grafts. With fine hair of 50-60 microns, the number rises almost double for the same area. In the temples, the target density is around 30-40 follicles per cm², compared to 50-60 for a well-populated forehead. There's no need to go beyond that: the natural effect is given much more by the hairline than by the quantity.
Then there's the type of follicular unit. In the temples, almost only single-hair grafts are used, because follicles with two or three hairs create an unnatural "tuft" effect and are kept for the forehead and crown. This raises the count: more single units are needed to achieve the same coverage. A plan that promises high density with few grafts, in this area, should be viewed with suspicion.
The state of the donor area also matters. With a good extraction, 60-70 units per cm², the temples can be filled without affecting the rest of the hair transplant. If the donor area is already thinning, the plan changes and the numbers drop. And the last factor, often the most overlooked, is the future progression of thinning. A 25-year-old patient with marked recession but still unstable loss doesn't receive the same plan as a 45-year-old man with a stable picture for a decade.
And those who fear the unnatural effect discover that the orientation of the grafts makes the difference: ten degrees of wrong angle are noticed much more than a hundred fewer grafts.
So, how many grafts for the temples in practice? Between 500 and 1200 in total, depending on how advanced the recession is and how much you want to correct. Any other number, without a consultation, is a blind estimate.
From one graft, how many hairs grow?
When we talk about "how many grafts for the temples," there's a fundamental misunderstanding to clarify right away. From one graft, only one hair grows. The graft is the root, the living part that produces the shaft. Period.
The confusion arises because in common language we call "grafts" what in medicine are follicular units, that is, small groups of follicles that come out together from the same pore. Each unit contains 1 to 4 hairs. On average, on an adult's scalp, a follicular unit carries 2-2.5 hairs. But at the temples, the proportion changes: single-hair units prevail, and this influences the entire count.
How follicular units are classified:
- 1-hair units: typical of the temples and lower nape.
- 2-hair units: the most common on the top of the head.
- 3-4 hair units: frequent in the central area and on the crown.
That's why the figures you hear in consultation should be read carefully. If the surgeon says "500 grafts per side," they're not talking about 500 hairs. In a central area, 500 units at an average of 2.2 hairs equal about 1,100 hairs. At the temples, however, 500 grafts can correspond to only 600-700 shafts.
So the real question isn't "how many grafts for the temples" in absolute terms, but how many grafts are needed to cover the area and what type of follicles are selected. An experienced surgeon chooses them one by one under the microscope. For the hairline, fine single units are used: a graft with 3 hairs placed there would form an unnatural bush.
In practice, to evaluate a quote: a small temple area requires 300-500 grafts per side. A larger area can reach 700-800. These are mostly 1-hair units, which is why the result looks delicate and natural.
3000, 4000, 6000 grafts: how many hairs are they?
On a healthy scalp, an average of 100,000 hairs grow. A transplant of 3,000 grafts does not bring back the entire head of hair: it reconstructs specific areas, usually the frontal hairline or the crown. To understand if the number is enough, it's best to think in square centimeters, not individual hairs.
Here, "graft" refers to the follicular unit, i.e., the group of 1-4 hairs that emerge from the same orifice. In the donor area, the occipital region, a unit contains on average 2.2 hairs. The math is easy: 3,000 grafts are worth about 6,600 hairs. 4,000, about 8,800. Numbers that say everything only if distributed well.
The numbers should be read with caution. Two patients with 4,000 grafts can leave the operating room with very different densities: it depends on the caliber and color of the hair, as well as the contrast with the skin. A thick, dark hair on light skin covers more than ten thin, blond hairs. Anyone evaluating a procedure should know this before signing.
Why a graft is not just one hair
The internal composition varies from patient to patient. Out of a hundred units taken from the back of the head, about ten have a single hair, about fifty have two, about thirty have three, and the remaining four have four. Those with rich units exceed the average of 2.2 hairs, while those with fine hair stop near 2. In the operating room, units are counted: the number of grafts determines the time and cost of the procedure. The result, however, depends on other factors.
The quality of the donor area weighs as much as the number. A fine occipital area with 6,000 units yields less than a robust one with 4,000. This is why the first consultation never ends with a quote: it ends with an analysis of the donor area.
What 3,000, 4,000, or 6,000 grafts really cover
A square centimeter of scalp under normal conditions hosts 70-80 follicular units. For a natural result, post-transplant density settles at 40-50 units per square centimeter. Below this threshold, the effect is sparse; above it, too much donor area is consumed.
With 3,000 grafts, you cover 60-80 square centimeters: forehead and hairline with a hint of crown. With 4,000, you reach 80-100 square centimeters, the classic range for a Norwood 4. With 6,000, you cover 120-150 square centimeters, the case of a Norwood 5-6. There is a physical limit to keep in mind: the donor area measures 200-250 square centimeters and provides 6,000-8,000 units over a lifetime, no more.

Ten to twelve hours in the operating room keeps follicles outside the body for too long and increases the risk of shock loss. How much it costs based on grafts
In Turkish lira, a transplant of 4,000 grafts ranges between 65,000 and 100,000 TL. When you request a quote, the first question to ask is how many grafts are needed: without that number, any figure is meaningless.
In 2024, I followed a 41-year-old patient with Norwood 4: 4,200 grafts in a 9-hour session, 2,400 on the anterior half and 1,800 on the crown. At three weeks, almost all grafts had fallen out, a normal shock loss. At ten months, the density was already good.
How many grafts for receding hairline: mild, moderate, and advanced
Determining how many grafts are truly needed for a receding hairline is not an abstract calculation. It depends on the degree of hair loss progression, the quality of the donor area, and the density you want to achieve. The most used scale remains the Hamilton-Norwood scale, which goes from stage I to VII: each step has a different graft requirement. A serious surgeon doesn't stop at the number; they also evaluate the caliber of the hair, its thickness, and the expected survival rate of transplanted grafts.
The numbers below are indicative. They come from a comparison of the main Italian hair transplant centers and data published in 2024. No quote written via WhatsApp can replace a visit with trichoscopy: the count of available grafts in the occipital area changes the surgical plan much more than the patient's aesthetic request.
- The density of the donor area: below 50-60 follicular units per cm², the yield is poor.
- The future stage of hair loss: a Norwood III at age 25 often becomes V by age 40.
- The actual surface to reconstruct, measured in cm² and not by eye.
Mild receding hairline: Norwood II-III, resolved in one session
The loss affects the temples and frontal hairline, often with a 45-degree entry that begins to curve. Between 1,800 and 2,400 grafts are needed to redraw a natural line without unnatural effects. This is the phase where intervening is most convenient: a small area requires few grafts, and few follicular units at high density yield much more than many spread over a large surface. A 33-year-old patient with Norwood III and a good occipital reserve closes the matter with about 2,000 grafts in a single 6-7 hour session.
Moderate receding hairline: Norwood III-IV, frontal and crown together
When the receding hairline also involves the crown and the vertex area, the requirement rises to 2,500-3,500 grafts. The area to cover is larger, and the grafts must be distributed wisely. Many surgeons choose not to lower the hairline as much as the patient requests, precisely to concentrate density where the aesthetic contrast is greater, i.e., in the frontal region. A patient with diffuse thinning on the upper area often finds that the crown repopulates with 800-1,200 grafts, while the frontal part absorbs twice that. This is a compromise that must be explained well beforehand, not discovered after the procedure.
Advanced balding: Norwood V-VII, in mandatory stages
Beyond the fifth stage, with the crown and top area completely exposed, you need 4,000 to 6,000 grafts. No honest clinic promises to complete the job in a single session: it proceeds in stages, with the first procedure dedicated to the front and a second after 10-12 months for the crown. The real limit is the availability of follicular units in the donor bank, not the patient's will. Planning starts from the residual density: if the back of the head offers less than 4,000 grafts, the surgeon must accept a sparser result and say so clearly during the consultation.

Shock loss between the sixteenth and twentieth day is normal, not rejection: the hair falls out, not the graft.
Anyone who has ever planted saffron bulbs in a pot already knows the principle: the quality of the bulb matters more than the quantity. The same logic applies to hair. Hair grafts taken from the back of the head survive for decades because they resist dihydrotestosterone, while those from other areas do not. That's why the donor area and the delicacy of FUE extraction matter more than the price per graft: that's where the success of a transplant at any stage is decided.
